They messed up or denied (at first) almost every claim submitted. They take forever to process their claims and their rules are confusing. If they don't have a rule excluding your claim, then they have processed the claims incorrectly and avoided paying me/the doctor when they should have. There are 2 deductibles - for in- and out-of-network care. My massage therapist was not in-network, but they applied an in-network deductible saying that all massage therapy was considered in-network. What? I submitted a claim for a naturopath. According to their policy they should cover it as a STD office visit, with no deductible. They messed it up 2x. Turned out that they "categorized" this doctor differently, as an acupuncturist, and therefore there would be a deductible applied before providing 60%. This was wrong; the claim was submitted correctly both times; it finally got corrected but not without a lot of effort. I'm still waiting on a claim to be processed 14 months later for a surgery and just found out that the claim is still being contested.
