Denied Because Of One Missing CheckmarkFeb 11 2013
One afternoon I bent over to pick something up off the ground. I ended up falling to the ground and could not get back up for at least 20 minutes. I had a friend drive me straight to the doctors. He wanted to do an MRI on my back to see what had happened. The doctor wrote me a note that said that I could not work for at least two full weeks and needed to seek physical therapy. I pay Colonial Life $150/month for accident coverage. My coverage was supposed to kick in on the first day out of work due to injury. After 12 days they denied my claim. When I inquired as to why, I was told that it was because my doctor did not check off "accident" on one of the forms that he submitted.
