say test is covered then deny when bill submitted

I have UHC through my wife's employer. My doctor wanted me to have Ctmylogram, I called UHC twice to make sure this was covered. Even told rep both times that i would not have test at this time if it was not covered. Both times the Reps. assured me the test was covered. The hosp also verified test was covered. After the bill was submitted they denied claim!
