coverage terminated monthly for no reason

Almost monthly I have had to re submit all student dependent paperwork to fill a script. After they are sent, received, and a week of time passes the issue is still not resolved. I'm going on my second month of false termination of coverage without medications. Daily I am on the phone with rude, unhelpful customer service people who claim I need to resend the packet weekly. Any hospital visit turns into a nightmare. In the past (this happens every other month every year since 2005) the issue has been fixed in about 2 weeks of time-- only with consistent reminders to the co. As stated before I'm going on the second month of waiting. Apparently we pay for a plastic card that does nothing. I just don't understand how this is legal. They should expect a lawsuit soon.
